Consent form reminder when getting jab

The Department of Health are remind people to bring their content forms before attending vaccination hubs.

Consent forms can only be signed by the patient, or if the patient lacks capacity then it can be signed by their GP, they cannot be signed by another person on the patients behalf – not even a close relative.

The individual receiving the vaccination needs to clearly understand the consent form – this will be checked at the hubs.
