Nurses on the Isle of Man are to be balloted over a proposed 4% pay rise offered by Manx Care.
Negotiations over the pay award have been ongoing since May last year.
The Royal College of Nursing has called for a 15% rise and a one-off payment for its 700-strong workforce.
Health Minister Lawrie Hooper will ask politicians to approve a £10m overspend in next month's Tynwald sitting.
The Union said: "Fair Pay for Nursing is about recognising the complexity of skill, responsibility and experience demonstrated every day our members and their colleagues as well as providing safe and effective care."
